Putting an alt tag on a thumbnail

I had a accessibility expert review the LibreTexts work I have been recently doing and she told me that all the thumbnails must include an alt tag since screen readers need it for accessibility requirements.  I completely understand how to create alt tags for images in the documents, but how can I create an alt tag for each of the thumbnails?

That is not 100% correct. Thumbnails are for decorative purposes only and hence are excluded from requiring alt-texts. However, the bigger issue is that the thumbnails use the title of the section as the alt-text and that isn't good (it should just be empty to signify a decorative image).

What is a related, but slightly different issue, is that we are working on removing the "title=" tag in the code so that the tooltips that are native to browsers are not active when the mouse hovers over the thumbnail. The browser tooltips do not satisfy modern WCAG requirements.

Another issue that she brought up was 
"All pages in this textbook should begin with a content heading (Heading 2) at the top. People
who use screen readers depend on accurate and complete heading lists to help them navigate
page content. Without an initial Heading 2, it’s possible to miss the first section of text."
Since LibreTexts is pretty uniform in its format, I think that all the pages have the same header style at the top for just about every page in the entire collection.  Do you have any comments that I can forward to her.

I don't think so. Starting the page with a H2 (right after the H1 title) isn't needed to my knowledge and hasn't come up with our a11y discussions. I tend to like text between the h1 title and a h2 section. Having the page set up like that isn't a bad idea, just not my personal preference. However, the first h2 shouldn't be a repeat of the title (in most cases).
